This is a metal update of the original plastic version. The plastic ones warp and leak so VW updated these to a metal unit. Location - this part is bolted on the cylinder head near cylinder #2 (passenger side towards the rear) and the 7mm hose comes off of it and up to the coolant rail that runs around the perimeter of the engine compartment.
Now included, gaskets and hardware! The Volkswagen part number for the outlet with the nipple is 025121160D.
